Tubes of choice (Close-up and Full Body), I am using one of Keith Garvey purchased from MPT, you can now find him at PTE
Scrap kit Beautiful Emotion by Lacarolita's Designz
Tag Template 19 by DBA
My Supplies (Mask and Word Art) here,  you can always use your own

Open the template, duplicate and close the original. Resize canvas by 700 x 700 and add a white background layer at the bottom.

Select the circle, go to Selections - Select All - Float - Defloat

Copy and paste as paper of choice above the circle layer. Go to Selections - Invert and hit delete on your key board. Apply a drop shadow or a gradient glow. Delete the template layer.

Repeat this for all template layers.

Copy and paste the word art in my supplies, place inside the middle rectangle.

Copy and the mask above the white background layer.

Copy and paste the close-up inside the frames, erase anything outside the frames. Apply a drop shadow.

Open Bow 9 (ribbon) and resize by 60 percent, go to Image - Rotate Right. Do Not Close. Copy and paste as a new layer. Move this layer below the top rectangle. Position to your liking, I overlapped the loop using Promote Selection to Layer. To do this select what you would like to bring to the top, then right click on it in the list of layers to the right, and choose the option I mentioned above. Now move this layer above the rectangle layer. Be sure to add a drop shadow BEFORE you so this step or it will not look right.

Open Bow 10 (ribbon) and resize by 65 percent. Copy and paste as a new layer placing on the left hand side of the frames. Apply a drop shadow.

Open Button 1 and resize by 15 percent. Copy and paste as a new layer at the top left hand side on top of the ribbon. Apply a drop shadow. Duplicate, move to the second frame in the lower right corner. Duplicate once more, mirror and place in the third frame on top of the ribbon in the lower left hand corner.

Open Bow 6 and resize by 30 percent. Copy and paste as a new layer. Place on top of the button in the second frame. Apply a drop shadow.

Open Flower 5 and resize by 12 percent. Copy and paste as a new layer. Place to your liking. Apply a drop shadow. Duplicate as many as you like.

Copy Bow 9 and undo the rotate, copy and paste as a new layer on top of the rectangle at the bottom of the template. Apply a drop shadow. Make sure this layer is below the frame layers.
Open String 1 and resize by 60 percent. Copy and paste as a new layer. Position at the bottom on top of the rectangle, make sure this layer is above the frame layers. Apply a drop shadow.

Open Glitter 1 and resize by 30 percent. Copy and paste as a new layer. Position to your liking, move this layer above the mask layer. Duplicate as many as needed to your liking.

Open Templante 2 (stars) and resize by 60 percent. Copy and paste as a new layer. Place at the bottom of the tag. Apply a drop shadow. Duplicate, mirror and flip. Reposition if needed.

Open Buckle 1 and resize by 25 percent. Copy and paste as a new layer. Place towards the right on top of the top rectangle. Apply a drop shadow.

Open Skull 1 and resize by 50 percent. Copy and paste as a new layer. Place below the buckle. Position so it looks like the skull is hanging off the buckle. Apply a drop shadow.

Add any copyright and your name.

Crop, resize if desired. Save as a PNG (close background layer first).
